REMINDER: CHILD CARE IMMUNIZATIONS REPORT DUE: 12/1/2023

As a reminder, Immunization Reports for child care centers and preschools are due to the Washington State Department of Health (DOH) by December 1, 2023. This is a requirement for all child care centers and preschool programs under WAC 246-105-060 and RCW 28A.210.110.

Child cares and preschools can use this PDF form – Child Care-Preschool Annual Report Worksheet (PDF) – to gather the information needed for the report.

The survey will close on December 1, 2023 and can be completed at the following link: Complete the online survey for the annual immunizations report.

Children who attend a child care program and are also in transitional kindergarten should not be reported by the child care. The immunization status of these children will be reported by their public or private school.

This chart provides the required immunizations for child care by age: Child Care Immunization Parent Chart (wa.gov). It is available in several languages here.
