Septic Application Forms

The following septic application forms must be submitted by a professional engineer or Washington State licensed septic designer. To understand more about our permitting process, visit the septic permit process steps page.

For properties served by an alternative septic system, including all system types other than a gravity or low pressure distribution system, the restrictive covenant below must be provided to the Health Department prior to building permit approval and septic system installation permit issuance. In addition, a monitoring and maintenance service agreement must be formed.

The covenant must be signed by the property owner(s) in front of a notary and recorded with the Snohomish County Auditor Office prior to submitting the document to the Health Department. The recorded document may be emailed, or it may be mailed or delivered in-person to the Environmental Health front counter:
